It is a hereby agreed and understood that there shall be not contract of Takaful coverage unless this appliction is accepted by DFTL and required contribution is duly paid. The DFLT's lability shall be governed by the terms & conditions of its group family takaful master policy.

 INSTRUTIONS
1 Make your cheque payable to "Dawood Family Tataful Waqf".
2 Both the hard -copy & soft -copy list of employees as indicated below are required. Please send the hard -copy list in duplicated duly signed by an authorized officer. One copy will be retained for our record whilst the other copy will be returned to you ater counter-signature.
3 The list should be prepared in descending order of seniority for eachDivision or Department with continuous serial number.
4 The list should indicate the emploment status for person whether permanent or under probation. Only regular and permanent employees my be covered under Group Family Term (GFT) or Group Personal Accident (GPA).
5 A permanent employee is a person who has been engaged on permanent basis likely to last more than nine (9) month , and has satisfactorily completed a probationary period of three (3) months. Those who comopleted the probation during the effective period of the policy shall be covered from such dated of their becoming eligible and contribution charged accordingly.
6 The proforma is a guide to prepare the information required of employees desired to be covered under GFT or GPA. A separate requirement for Group Credit Protection has been provided as below.
